RegionalNeeds.org was an interactive Community Needs Assessment sponsored by the United Way of Greater St. Louis. The CNA spanned 16 counties across two states (Illinois and Missouri). The CNA served the St. Louis community as a foundation to better understand what challenges people in the region are facing. The report also highlighted the most common needs across the Greater St. Louis region.

The Well Connected Communties portal served as a resource for more than 39 communities across 17 states who were working towards meeting healthy living goals. Managed by CARES, the Well Connected Communities Hub provided grantees access to a robust Community Health Needs Assessment tool, County Health Rankings reports, quarterly reporting forms, resources from the field, and archived professional development events.

The SNAPEd National report provided data related to SNAP eligibility, SNAP participation, income and poverty levels, and other social determinants of health data.

Partnership for a Healthier America logo

Partnership for a Healthier America (PHA)

Partnership for a Healthier America previously hosted both a Hub and a Widget on the CARES Network. PHA is focused on encouraging healthier eating and lifestyles for children and young adults by improving local food environments. The ImPHAct Map, previously available on the PHA website, collects and displays information on corner stores and campuses who have committed to be PHA partners.

Creating Healthy Communities Ohio

The Creating Healthy Communities Ohio Hub was launched in 2015 by the Ohio Department of Health. The CHC Ohio Hub supported 20+ counties in Ohio with health-related policy, system, and environmental change strategy tracking, evaluation, and reporting. The Hub helped individual counties track progress on community health interventions and provided aggregate data to state-level managers for initiative-level metrics.

Environments Supporting Healthy Eating (ESHE)

The ESHE Index was developed in partnership with the Council on Black Health in 2015. The index ranked counties across the United States on factors related to healthy eating. The goal of ESHE Index was to stimulate changes in food and food policy landscapes that can facilitate high dietary quality. The ESHE index was discontinued in 2021.

ExploreMOHealth was launched in 2018 with support from the Missouri Hospital Association and the Health Indicators Data Institute (HIDI). ExploreMOHealth provides zip-code level rankings across major health behaviors and health outcomes in the state.
