New 5-year estimates for 2008-2012 acquired from the American Community Survey (replacing 5-year estimates for 2007-2011). All indicators using data from the American Community Survey have now been updated to use the latest release. Affected indicators:

Maps and data for heart disease mortality (all) from the Centers for Disease Control and Prevention (CDC) National Vital Statistics System (NVSS) has been added to the CHNA platform. Heart disease mortality is defined by the following ICD-10 codes: I00–I09, I11, I13, I20–I51. New Indicator:

New chronic disease indicators added to the CHNA platform!

Maps and data for chronic disease among Medicare beneficiaries from the Centers for Medicare and Medicaid Services (CMS) have been added to the CHNA platform. Indicators report the prevalence of heart disease, diabetes, high blood pressure, and high cholesterol in the Medicare fee-for-service population. New Indicators:
