The New York State Community Action Association Data Hub is designed to assist local Community Action Agency staff in the development of Community Needs Assessments (CNAs). The NYSCAA Data Hub provides access to core secondary indicators for assessing and addressing poverty. The Creating Healthy Communities Ohio Hub was launched in 2015 by the Ohio Department of Health. The CHC Ohio Hub supports 20+ counties in Ohio with health-related policy, system, and environmental change strategy tracking, evaluation, and reporting. The Hub helps individual counties track progress on community health interventions and provides aggregate data to state-level managers for initiative-level metrics.

In 2018, the Georgia Rural Health Innovation Center and Mercer University partnered with CARES to provide the state of Georgia and its public health entities with a custom community health needs assessment (CHNA), Map Room, and a Health Asset Finder. It hosts the Missouri Map Room and tools to support University of MIssouri Extension programs and services.
AgSite provides targeted information about property and farms across the state of Missouri. The AgSite report include data related to weather conditions, soil health, and native and threatened species. AgSite is designed to support risk assessments, land purchases, and guide investments.
