The North Carolina Data Portal was launched in 2024 to provide data, maps, and tools to support community health assessments and other public health activities in North Carolina.
Andrew County Local Public Health Department Reportable Disease Dashboard
The Andrew County Public Health Department has teamed up with CARES to develop a local public health dashboard that provides citizens and the media with easy access to data for all major reportable diseases. Data are shown for more than 15 reportable diseases in the county with benchmarks for previous years.
Center for Health Policy
CARES provides data analysis support to the Center for Health Policy at the University of Missouri School of Medicine.
Massachusetts Health Data Tool
The Massachusetts Health Data Tool is a suite of reporting and mapping platforms designed to help visitors find community- and state-specific data quickly and easily. With access to over 100 indicators, the Community and State Reporting tools provide access to up-to-date data on social determinants of health, as well as health behaviors, health outcomes, and healthcare delivery. In the Map Room, visitors can map their communities and explore over 28,000 current and historical data layers from more than 120 national secondary sources.
Adventist Health PAIR Tool
The Adventist Health Priority Area Indicator Ranking (PAIR) tool is designed to help the Adeventist Health Community Benefit Team create, enhance, and activate community health needs assessments and interventions. The PAIR tool helps hospitals prioritize the individual characteristics that need the most attention and the high-level health category that needs addressed the most at the community level.
Boone County Food Insecurity
Boone County Food Coalition brings together local stakeholders to address food insecurity in the Boone County area. CARES provided data acquisition and mapping services to the Coalition.

The Population Health Assessment Engine (PHATE) provides an investigative tool for physicians, hospitals, and clinics to visualize neighborhood characteristics and patient health risk factors in their service area (by Census Tract). The tool integrates data from the American Board of Family Medicine’s (ABFM) Prime Registry database, but registers users can also upload information to visualize service area characteristics.

Trinity Health
The Trinity Health Data Hub supports a network of hospitals and their community benefit teams by providing easy access to core health outcomes and behaviors. The Hub hosts two reports – a custom Community Health Needs Assessment and a Vital Sign report. Data are available by hospital service area, aggregate areas, and system-wide.
