Locations of Primary Care Physicians

This layer displays the locations of all primary care physicians who accept Medicare. Primary care physicians are those MDs or DOs specializing in family practice, general practice, pediatric medicine, or internal medicine. Data are from the Centers for Medicare and Medicaid Services (CMS) Physician and Other Supplier 2012 Public Use File (PUF), which was released in April 2014, and from the 2013 CMS Physician Compare National Downloadable File.

Head Start Program Locations

This layer displays locations of Head Start facilities in the United States. The Administration for Children and Families (ACF) identifies head start facilities as either a center, an early childhood center, a seasonal / migrant center, or any combination of these designations.

Majority-Minority Schools

This layer displays the location of public schools where the majority (50% or more) of the student body is a minority race or ethnicity (American Indian, Asian, Black, Hispanic/Latino, Pacific Islander, or Multiple Race). This point-level data is acquired from the 2012-2013 National Center for Education Statistics (NCES) Common Core of Data (CCD).

Urban Rural Classification Scheme by County, 2013

This dataset is used to study the associations between urbanization level of residence and health and to monitor the health of urban and rural residents. NCHS has developed a six-level urban-rural classification scheme for U.S. counties and county-equivalent entities.

Health Insurance Market Place Enrollment, 2015

The dataset provides the total number of health plan selections by ZIP Code for the 37 states that use the HealthCare.gov platform, including the Federally-facilitated Marketplace, State Partnership Marketplaces and supported State-based Marketplaces. These data reflect the total number of consumers who selected a plan or were automatically re-enrolled for the 2015 coverage year as of January 16, 2015.

The dataset does not include plan selections from the District of Columbia and 13 other states that have State-based Marketplaces.

Data for ZIP Codes with 50 or fewer plan selections are not included to protect individuals’ privacy.

PRISM 30-Year Average Temperature, 1981 – 2010

This layer depicts average annual temperature based on data from 1981 to 2010. Data was obtained from the PRISM Climate Group at Oregon State University. PRISM models climate information using a process which incorporates a digital elevation model. Monthly average data is also available.

National Transporation Atlas, 2014

Multiple layers from the 2014 release of the National Transportation Atlas (NTA) are now available through Community Commons. The US Department of Transportation releases this annual dataset displaying many transportation related features, ranging from highway and airport locations to fatality accident locations and hazardous material transportation routes. Layer displays the location of Food Hubs across the US. Data was obtained from the USDA Local Food Directories on January 7, 2016

Income from Veterans Benefits, 2013

Layer displays information about transfer payments income from veterans’ benefits from the Bureau of Economic Analysis (BEA) Local Area Personal Income data series. Additional attributes include transfer benefits per capita, and percent change in benefits between 2009 and 2013.

Physical Inactivity by County, 2013

Layer displays the estimated percentage of adults who do not participate in any leisure-time physical activity. These figures are multi-year modeled estimates based on survey data from the CDC Behavioral Risk Factor Surveillance System.
