The Virginia Department of Public Health and the Virginia Hospital Association have partnered with CARES to build a Community Health Needs Assessment tool to support and align public health departments, hospitals, community health centers, and others across the state in meeting targeted health goals.

Missouri Vocational Rehabilitation Portal

The Vocational Rehabilitation Hub facilitates the recordkeeping of Pre-Employment Transition services provided by trained educational specialists throughout the state of Missouri. Services fall into one of the following categories: Job Exploration, Work-based Learning, Post-secondary Counseling, Workplace Readiness and Self Advocacy. Using the Hub, Specialists record service(s) provided to individual students on a daily basis. Services are aggregated and sent to the state Vocational Rehabilitation office every month to support their federal reporting mandate.

Kentucky Vocational Rehabilitation Portal

The Kentucky Vocational Rehabilitation Hub supports ETS specialists (employment transition services) in entering data and reporting about students’ use of services. These data are aggregated to meet federal reporting requirements and used by staff at the Kentucky Career Center. Data are stored in secure databases due to sensitive information.

Missouri Source Water Assessment Portal

The Missouri Source Water Assessment Portal provides web-based access to information about the safety of drinking water in Missouri. Specifically, the portal provides information on drinking water sources, such as wells, rivers, lakes, and springs, and potential sources of contamination that put drinking water at risk.

Washington State Community Action Partnership

The Washington State Community Action Partnership Data Hub is designed to assist local Community Action Agency staff in the development of Community Needs Assessments (CNAs). The WSCAP Data Hub provides access to core secondary indicators for assessing and addressing poverty. The Hub also provides Washington CAA’s with access to county-level COVID-19 data.

Time’s Up Measure Up

Time’s Up Measure Up is an multi-organization initiative from the Time’s Up foundation to combine quantitative and quantitative data to expose economic and other hardships facing women as a result of the COVID-19 pandemic and beyond. CARES performed data consultation to identify quantitative datasets to represent the TUMU conceptual framework. CARES then acquired data for 15+ metrics, relying on knowledge of existing secondary sources as well as custom survey data analysis.

University of Missouri Impact Portal

The UM Impact Portal reports highlighting the impact of the University of Missouri presence across Missouri. The Impact Report provides both aggregate and county-level university-specific statistics like the number of employees, students, and alumni in an area.

Public Health Assessment Engine (PHATE)

The Population Health Assessment Engine (PHATE) provides an investigative tool for physicians, hospitals, and clinics to visualize neighborhood characteristics and patient health risk factors in their service area (by Census Tract). The tool integrates data from the American Board of Family Medicine’s (ABFM) Prime Registry database, but registers users can also upload information to visualize service area characteristics.

RocHealthData

RocHealthData is a collaborative data-sharing Hub focused on serving the greater Rochester region in New York. Powered by local partners and the University of Rochester, RocHealthData is collecting and sharing local data internally through the Hub, and externally with public stories and maps.

Health Equity Report Card, powered by CARES, provides information on how your county is doing on a variety of health-related conditions compared to the rest of your state and nation. The data show how your area stacks up in housing, transportation, poverty, healthcare, mental health, environmental issues, and access to healthy food and active spaces.
