ADD INFO ABOUT GLOBAL VIEWER – The Population Health Assessment Engine (PHATE) provides an investigative tool for physicians, hospitals, and clinics to visualize neighborhood characteristics and patient health risk factors in their service area (by Census Tract). The tool integrates data from the American Board of Family Medicine’s (ABFM) Prime Registry database, but registers users can also upload information to visualize service area characteristics.

Environments Supporting Healthy Eating (ESHE)

The ESHE Index was developed in partnership with the Council on Black Health in 2015. The index ranked counties across the United States on factors related to healthy eating. The goal of ESHE Index was to stimulate changes in food and food policy landscapes that can facilitate high dietary quality. The ESHE index was discontinued in 2021.

The Lehigh Valley Community Hub has been a CARES partner for 10+ years. The Lehigh Valley Hub supports leaders working toward community change with a focus on cross-sector data-driven collaboration. The intent of the Hub is to encourage the sharing of data and the use of data to effect change throughout the lifespan of an initiative.

Missouri Food Assessment

The Missouri EATs Assessment Tool provides critical food and social determinants of health data to communities in Missouri. These data are used to begin conversations and take actions aimed at strengthening community food systems. CARES began hosting the Missouri EATs Assessment Tool in 2018.

NMTracker

NMTracker (Nutrient Management Tracker) is suite of online tools that support tactical nutrient management planning for farmers and ranchers. NMTracker supports mapping and data entry for nutrient management planning and integrates with the Purdue University Manure Management Planner (MMP). CARES has hosted NMTracker since 2011.
