As part of the 2025 data release, all county-level estimates have been updated using the CDC’s latest methods, and historical data have been recalculated back to 2004 to ensure consistent comparisons across years.
Updated CDC obesity and physical inactivity maps and data now available (2004-2023)

Diabetes Among Medicare Beneficiaries by County, 2017
This layer displays the percentage of Medicare Beneficiaries with diabetes. The prevalence of chronic conditions among Medicare beneficiaries is based on Medicare insurance claims (for specific inpatient diagnosis or procedure codes) in 2017.
Diabetes Mortality for 2014-18 by County
This layer displays diabetes mortality rates from the CDC’s National Vital Statistics System – Mortality database. Data are 5-year aggregates for 2014-18 and grouped by ICD-10 codes.
Adults with Diabetes by County, 2017
This updated layer displays the estimated percentage of adults with diagnosed diabetes by county. These figures are multi-year modeled estimates based on survey data from the CDC Behavioral Risk Factor Surveillance System.
