In 2018, the Georgia Rural Health Innovation Center and Mercer University partnered with CARES to provide the state of Georgia and its public health entities with a custom community health needs assessment (CHNA), Map Room, and a Health Asset Finder. These tools were all launched as widgets, or embeddable tools, and can be found on at https://georgiaruralhealth.org

Grazing Wedge Map Widget

The Grazing Wedge tool is designed to provide farmers with best practices for forage management, including strategies for forage optimization by measureing forage growth and rotation. The Grazing Wedge tool allows users to draw farm and paddock layouts, enter data, and visualize forage status. This is a collaborative project between CARES and Extension Technology & Computer Services (ETCS).

Community Action Association of Pennsylvania (CAAP)

The CAAP Data Hub is designed to assist community action agency staff with the development of Community Needs Assessments (CNAs). Launched in 2015, the CAAP Data Hub provides access to core secondary indicators for assessing and addressing poverty. The Hub also provides Pennsylvania CAAs with access to local data, maps, and specialized resources.

Community Action Partnership Association of Idaho

The CAPAI Data Hub is designed to assist local community action agency staff with the development of Community Needs Assessments (CNAs). Launched in 2019, the CAPAI Data Hub provides access to core secondary indicators for assessing and addressing poverty. The Hub also provides Idaho CAAs with access to county-level poverty reports.

Creating a Culture of Health in Appalachia

The Health in Appalachia Hub, sponsored by the Appalachian Regional Commission and launched in 2018, is designed to help identify economic and health issues in the Appalachian area. The Hub features compelling “Bright Spot” case studies and maps on health outcomes and health factors (9 domains and 41 indicators).

All Things Missouri

All Things Missouri was launched in 2018 and quickly became Missouri’s central data and mapping resource for policy-makers, business owners, farmers, and community organizations working to resolve issues related to: agriculture and environment; business and community; health and safety; and youth and family. All Things Missouri provides access to national, state, and local (Missouri-sourced) data all in one platform. It hosts the Missouri Map Room and tools to support University of MIssouri Extension programs and services.

Missouri Food Assessment

The Missouri EATs Assessment Tool provides critical food and social determinants of health data to communities in Missouri. These data are used to begin conversations and take actions aimed at strengthening community food systems. CARES began hosting the Missouri EATs Assessment Tool in 2018.

Show Me Food

ShowMeFood.org facilitates the sharing of information for and about farmers and local food providers in the state of Missouri. Food-related businesses in Missouri can register and be placed on the map. Businesses can promote specific products, purchasing options, days of operation, and accepted payment types as part of their profile. ShowMeFood.org was launched in response to the effects that COVID-19 caused to state, local, and national food supply chains and distribution systems, and updated with a new design in September 2022.
